IMPORTANT: Before beginning your intake forms, make sure you have these items prepared to upload. 1) photo of your Florida drivers license or ID card to upload and 2) Proof of Condition. You will upload these within the Patient Portal. See Step 2 below for Proof of Condition requirements.

Please note, your address MUST match your FL drivers license or FL ID before completing the state’s application process. Please visit the FL DMV to make any necessary changes now. We request a copy of your license or ID for our records. You are responsible for uploading the required documentation for proof of residency to the state.
DISCLAIMER: By submitting and signing our Patient Intake forms you acknowledge you are a patient of Medical Marijuana Clinic, not the physician you will see at your scheduled appointment. If at any time that physician is no longer associated with Medical Marijuana Clinic, you will be activated in the Medical Marijuana Clinic’s medical director’s registry account.

After your forms are submitted, you will be redirected to our online scheduler.
Select your preferred location and New Patient service. Submit your appointment fee and wait for the appointment confirmation screen to ensure booking.

CANCELLATION POLICY: We are committed to providing exceptional care. Unfortunately, when one patient cancels without giving enough notice, they prevent another patient from being seen. To avoid a $30 cancellation fee, just text us at (813) 774-3474 before 10:00 am the day prior to your appointment or the Friday before if your appointment is on Monday.
