AFTER COMPLETING STEP 1, CONTINUE TO STEP 2. There is NO preapproval of your POC or waiting period to see if you qualify. Only the doctor can make the approval at the time of your appointment.
What to email? Please provide any record from your medical provider that shows you have been diagnosed for the condition you require medical marijuana for. The proof can be a simple medical record showing your diagnosis or diagnostic code, a copy of a prescription, a photo of your medication bottle(s) showing the medication name, prescribing doctor and your name, or an MRI/x-ray, etc.

Complete our online Patient Intake forms. After submitting your forms, continue on to Step 3 below to Schedule your Appointment.
Once you begin completing the forms, you must complete them. Any info entered will be lost if you exit the program. The Patient Intake forms will be automatically submitted to us when you click the ‘Done’ button.

Please note, your address MUST match your FL drivers license or FL ID before completing the state’s application process. Please visit the FL DMV to make any necessary changes now. We request a copy of your license or ID for our records. You are responsible for uploading the required documentation for proof of residency to the state after your doctor’s appointment and approval.
